Please complete this Modality Change/Addition Request Form for all change requests made after August 1, 2025.
This form allows for automation of the Modality Change/Addition requests so that CITOL will be able to track requests efficiently.
Requests require a file upload of an accessible syllabus for the course. If you are requesting a HYBRID Modality Change/Addition, you will download the document from the link provided within the form, complete the information requested, and then upload the document to the form.
Once the request is processed, you will receive an email with a “Score Release,” which is to serve as notification that CITOL has completed the process.
The “MCA Signature Form” will then be sent through Hello Sign. While the signature document does require the Registrar to sign, it does NOT automatically add the course to the schedule.
The course will need to be added to the appropriate semester schedule using the normal documentation sent to the Registrar’s office.
