Process for Accessing or Reviewing Student Health Records
- Restricted Access to Health Records
Program faculty and staff are not permitted to access or review student health records except for immunization and screening results with written permission from the student. These records are confidential and protected by privacy laws (e.g., HIPAA). - Handling of Health Records
Student health records are managed solely by designated health services or medical professionals. Students upload immunization and screening results in EXXAT. - Process for Obtaining Permission of Records
- Once uploaded into EXXAT, students can approve or disapprove release of results in EXXAT by choosing the desired option in the drop-down menu. Selected faculty and staff such as the Admissions Coordinator and Clinical Coordinator may view immunization and screening results to ensure students are meeting admissions and clinical site requirements, respectively. Students can also approve or disapprove release of results to clinical sites for Supervised Clinical Practice Experiences.
- Privacy Enforcement
Any violations of this policy, including attempts to access student health records, will result in disciplinary action in accordance with program, institutional and legal regulations.
