If you are requesting a HYBRID Modality Change\/Addition, you will download the document from the link provided within the form, complete the information requested, and then upload the document to the form.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Once the request is processed, you will receive an email with a &#8220;Score Release,&#8221; which is to serve as notification that CITOL has completed the process.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The &#8220;MCA Signature Form&#8221; will then be sent through Hello Sign.
